Scars & Repair Techniques

Scars Basics

Scars are areas of fibrous tissue (fibrosis) that replace normal skin after injury. Scar tissue is the same protein (collagen) as the tissue that it replaces, but the fiber composition of the protein is different; instead of a random basketweave formation of the collagen fibers found in normal tissue, in fibrosis the collagen cross-links and forms a pronounced alignment in a single direction.

There are several different kinds of scarring, each caused by differing amounts of collagen over expression. Hypertrophic scars occur when the body overproduces collagen, which causes the scar to be raised above the surrounding skin. Hypertrophic scars take the form of a red raised lump on the skin. They usually occurs within 4 to 8 weeks following wound infection or wound closure with excess tension and/or other traumatic skin injuries.

Keloid scars are a more serious form of scarring, because they can grow indefinitely into large, tumorous (although benign) neoplasms. All keloid scars are hypertrophic, but "only a small percentage of large scars" are keloid. Keloid scars can occur on anyone, but they are most common in dark-skinned people. They can be caused by surgery, an accident, by acne or, sometimes, from body piercings.

An atrophic scar takes the form of a sunken recess in the skin, which has a pitted appearance. These are caused when underlying structures supporting the skin, such as fat or muscle, are lost. This type of scarring is often associated with acne, chickenpox, other diseases [especially Staphylococcus (or MRSA) infection], surgery or accidents.

Stretch marks (technically called striae) are also a form of scarring. These are caused when the skin is stretched rapidly.

eTwo combines the unique Sublime and Sublative applications for comprehensive facial rejuvenation in as little as a single treatment:

  • Sublime™ uses the elōs combination of safe and effective levels of infrared light and bi-polar radio frequency energies to remodel the deep dermal layer of the skin by stimulating new collagen growth.
  • Sublative™ fractionated bi-polar radio frequency technology places the heat energy effectively into the upper dermis where it can produce a significant increase in both collagen and elastin with minimal epidermal disruption.

Persistent and untreated inflammatory acne lesions may cause various degrees of permanent scarring. Laser resurfacing is one of the most effective ways of reducing and clearing acne scars. Available laser treatments include ablative and non-ablative skin resurfacing. The choice of treatment depends on the type of scar (box, rolling, or icepick) and on the patient's preferences and expectations in terms of number of treatments and downtime.

Ablative resurfacing: ideal for patients seeking a dramatic improvement in a single session. It is also the most effective treatment on deep scars.

Non-ablative resurfacing: ideal for patients who prefer a milder treatment approach that requires several treatments but has less downtime.

Lumenis® CO2 ablative resurfacing: Lumenis' ablative technologies dramatically decrease scars, even very deep ones, as well as repair damaged skin and improve skin tone. Treatment is free of complications and requires minimized downtime due to the unique pulse technologies and tailored treatment modes. The ablative technologies are available in the UltraPulse® and AcuPulse™ products.

The Lumenis ResurFX™ is the only true Fractional Non-Ablative solution on the market. It uses an advanced scanner technology called CoolScan™. CoolScan uses a sophisticated, patent-pending algorithm for non-sequential scanning which ensures that the energy delivery is precise and homogenous, so the required coverage is achieved in only one pass. The ResurFX™ module is available as part of the M22 multi-application platform.

1320 Nd:YAG handpiece on HarmonyXL is used to treat fine lines, wrinkles and photo-aged skin, as well as mild acne scars. When used to treat aesthetic imperfections - including periorbital and perioral wrinkles - the 1320nm wavelength achieves deep penetration and targets selected water-containing tissue by raising the dermal temperature.

Pulse duration: 30, 40, 50 msec

Spot size: 6 mm

Energy density: 5 – 40J/cm2

Unlike surgery, laser scar treatment uses short pulses of micro-fine laser light to reach deeply into the skin’s sub-layers, treating the support structure. Then the body’s natural healing process sweeps away older, damaged tissue and rebuilds it with fresh, new collagen and elastin—the crucial building blocks of normal-looking skin.

PicoSure* is the world’s first aesthetic picosecond laser, widely recognized for its technology leadership, unmatched clinical versatility and proven performance. Recent PicoSure innovations such as Boost™ Adjustable Pressure and FOCUS™ Lens Array have enabled clinical applications and revenue expansion opportunities not available with any other laser system. Focus magnifies the PicoSure pulse 20X, creating laser induced optical breakdown (LIOB) in the epidermis. Studies have shown resulting new collagen and elastin production without the side effects and downtime of fractional and ablative lasers.

Available with Icon, the 1540nm Fractional, non-ablative laser for skin resurfacing and the treatment of surgical scars, acne scars and stretch marks – the only FDA cleared laser for this treatment. The various Microlenses allow for customized treatments; the XF Microlens for high speed, quick treatment sessions or the XD Microlens for deeper penetration to reach tough conditions such as scars and stretch marks.

Acne is a common skin condition that affects approximately 80% of adolescents 5% of adults usually fading without long-term damage. Regretfully however, some are left with disfiguring scars. Pollogen has developed TriFractional technology a clinically proven solution that safely and effectively minimizes acne scars and improves skin surface. Treatment is completely surgery-free and suitable for all skin types. Best of all, no down time is required. You can immediately return to your daily routine after treatment.

Pollogen's TriFractional technology stimulates the body to produce new collagen thus improving the skin’s texture and feel. During treatment, hundreds of small beams of RF energy penetrate the top layer of the skin, known as the epidermis, in micro-impact zones. The damaged acne skin cells are removed and the body produces new and healthy skin. The Maximus system is the most complete anti-aging system on the market. The Maximus provide multiple proprietary technologies, TriLipo RF, TriLipo DMA and TriFractional.

The Surgen is a medical-grade, non-surgical aesthetic platform powered by Pollogen’s revolutionary Hybrid Energy and TriFractional technologies for dermal volumizing and skin resurfacing. Results are enhanced over time and long-lasting.

Lasers are ideally suited for the effective treatment of a wide range of scar tissues. For hypertrophic scars, clinical research conducted with the Nd:YAG wavelength has demonstrated substantial improvements in the aesthetic appearance and overall tissue quality of a scar, including erythema, volume, pliability, etc. In many cases, further improvement can also be obtained using gentle ablative Er:YAG skin resurfacing. Acne scars in particular respond well to Er:YAG treatments.

With Fotona’s pioneering dual-wavelength scar-revision therapy, both nonablative Nd:YAG and fractional-ablative Er:YAG wavelengths are combined as part of a single comprehensive scar-treatment regimen, with immediate visible results that surpass that of traditional laser-based and conventional scar treatment techniques.

The SP Line of multi-application laser systems from Fotona (SP Dynamis & SP Spectro) provides a single, uniquely capable and full-featured solution offering the power of the industry's highest performance Er:YAG and Nd:YAG lasers, ideal for scar-revision treatments and a wide range of other applications in aesthetics and dermatology. The SP Dynamis also includes an additional surgical QCW Nd:YAG laser for lipolysis, hyperhidrosis and many other surgical procedures.
